Sudden acceleration

Sudden acceleration
My sister reported an issue with her 2011 G37 sedan. It has suddenly revved the engine and accelerated forward while braking 3l times in the past week, nearly causing an accident. I have done some research and have found only a couple of questions on the myG37 forum, the last of which was in 2014.
No doubt someone will ask, very reasonably, if it was driver error. It was not; nor was it a loose mat. It happened to both my sister and my niece; both are experienced drivers. The near-accident was averted only because my niece was able to exert enough pressure on the brake to stop in time, while parking.
I spoke with the Infiniti tech at the dealership and suggested that he re-flash the ECM and TCM, per a TSB from 2012. I also considered that it might be a throttle position sensor or some other control, or even the transmission. No codes have been stored, and the dealer, being unable to re-create the prblem, says that everything is OK. Obviously, my sister is freaked out that it might happen again and cause a collision.
Any ideas? Thanks!

You might want to remind your sister that you can always put the car in neutral while braking.
That should be much less stressful and might give her some peace of mind, if it happens again.
It isn't likely to be a throttle position or accelerator pedal position sensor, as each sensor has TWO sensors and the ECU checks both signals to make sure the signal is valid.
It is a good idea to get the ECU/TCM updated to the latest version.

There was a issue with the earlier ('08/'09) models where, under braking, the engine idle would "lunge," causing the car to "jump" forward. Infiniti even issued a TSB for the issue. It happened to me and I almost rear ended someone. The ECU refresh solved that issue
While not identical to the "sudden acceleration" you describe, it probably would be wise to have the ECU reflashed/updated.

Thanks so much. I think the issue is the same. My niece was braking while parking when it happened. I spoke with the Infiniti tech, and he will update the ECM and TCM. Thanks again!

Is your sister the sole driver of the car. It;s possible she had it on DS mode which revs up during braking to rev-match.
Try it again and see if she's on DS mode.

This was my first thought. She hit the brakes in DS mode, the transmission downshifted and rev matched and startled her. Definitely drive the car and see if you can replicate the issue
