I am planning to try this with my iPhone 5, since Apple recently released a HDMI out and VGA out adapter for the new lighting type connector.
Apple Store now has Lightning to HDMI and VGA adapters at $49 a pop, souped-up iPad charger

- I've never tried mirroring or video out on any iPhone. I thought it natively mirrored when plugged in with HDMI or VGA. Does the phone need to be Jailbroken and a special app installed?
- What is the phone dock you guys have in the pictures, the one that attaches to the right of the CD tray?

I have the HTC Evo 3D and have it hooked up the same way everybody else does in my 2009 G37 Coupe and I am really disappointed in the quality of the picture. Do I have my output mode or something wrong on my phone? Earlier I read that when the HTC was connected it went into tablet type mode, but mine doesn't do that. I'll post a picture in a second.
i have the same exact issue. very disappointed in the picture quality

The phone holder in my screenshots is from Proclipusa. All-together, costs about $75 (if memory serves), but it is a fantastic holder. Both vehicle specific and phone specific, so it is sturdy and looks great!
As for the sound, I use a Blackberry Bluetooth Receiver (for A2DP), and only use the video rca from the converter box.

In case anybody was wondering, here is my parts list:
Both the MHL cable and the Converter box will require a power source, so I purchased this high output, dual usb car charger.
This cable will allow me to (try to) put everything in the armrest, and only have a single cable coming out of the armrest to my phone. This cable is responsible for converting my phone's micro USB output to HDMI.
This is the converter box that is needed to convert my phone's digital signal (HDMI) to an analog signal (composite) that the G37 can use.
UPDATE:
This is another option for converter box recommended by dcg37x, on page 3 of this thread. It is smaller, cheaper, and doesn't require a power source. I would recommend this one over the previous.
This power cable SHOULD allow me to power the converter box using the aforementioned USB car charger.
This cable will supply power over the MHL cable, thus charging my phone when it is plugged in.

I am toying around with the option of using my phone with the oem screen or putting in the 7" Pioneer touch screen / Nav HU from my last car...

Let's see if we can't figure this out. I have an S5, bought the MHL-to-HDMI cable and converter box, and hooked it all up.
First trial in the house: phone to converter box via MHL-to-HDMI cable and converter box to TV via RCA. As expected, the quality was reduced dramatically but I had both audio and video displayed on the TV. I noticed however that the MHL-to-HDMI cable got really hot when everything was plugged in.
Second trial in the car: phone to converter box via MHL-to-HDMI cable and converter box to car screen via RCA. The video came on but when I tried to play music, the music would come from my phone and not the car stereo system. NOTE: I use the 3.5mm plug to the audio RCA plugs with no problem
Third trial in the house: to ensure that I really heard audio through the TV, I went back to the house and did another test through the TV again. I verified that indeed audio was coming through the TV speakers but this time, the video would NOT display on the screen. All I got was a very bad distorted block of static.
It was late so I called it a night but at this point, I seriously have no idea what the problem could be. I will try again after work today in the car. I highly suspect that it's the converter box but am open to any suggestions anyone may have.
Below are some pictures that were taken for the trial run. Please let me know if you guys have any thoughts.
